Look up + Conditions

Hi Gurus,

I would like to seek your expertise once again.
I'm having trouble of getting the right value for 2 conditions: Records/Hour and Hours

I want Records/Hour to have a specific value from a range and will reflect on the Incentive field.

On the Incentive field. I want the value depending on the Records/Hour to be multiplied by the value shown on the Hours.

If in any case there is one 0 as value to any of the category I want the Incentive field to say "No Incentive"
