Help creating a formula that will use countif with merged row on different tab

Hi,

I have survey template i need to create, i need to total the results of my survey everyday. Can someone please help me create a formula that will:

A) when you type a reason code letter in the daily survey tab, it will automatically input the reason in the next cell.

B) count how many times for each person Boxing or Hiking was chosen in the daily survey tab and list it in the survey results tab.

For (B) I'm not clear exactly what you want. For example on the DailySurvey worksheet does John Smith get a chance on 6/26/18 to opt for "Hiking"? In that case would you expect SurveyResults!D10 to be 1 and SurveyResults!E10 to be 1?

Your 6/26 DailySurvey template omits the "Code" column - I assume that's a mistake?

On the DailySurvey worksheet would it be possible to give "Date" it's own column? Doing so may make it easier to use pivot tables to generate your required summaries.
