Need a Formula to copy data to different cells based on a column header

I'm stumped on finding a formula to make this work. I need to be able to get the data from the top picture into the bottom by only entering a date into the cell on the second picture. The problem that has me stuck is that I need to be able to change the date and have the corresponding data that goes with the date come with it. For example, I need to be able to enter a date into Cell B 39, and have a formula take all the data listed under that date and place it into the correct areas. Attached is how the data is currently set up, any suggestions/help would greatly be appreciated

A picture (or two) is not much use for us to work with - it would be better if you attached a sample Excel workbook.

To do this, click on Go Advanced (below the Edit Window) while you are composing a reply, then scroll down to and click on Manage Attachments and the Upload window will open. Click on Browse and navigate to (and double-click) the file icon that you want to attach, then click on Upload and then on Close this Window to return to the Edit window. When you have finished composing your post, click on Submit Post. Don't try to use the Paperclip icon (Attachments button), as it doesn't work on this forum.
