Formula to lookup data contained in a matrix

Evening all,

I have a matrix and I need to return the relevant rate based on various criteria selected. Two of the criteria are held vertically in rows and one horizontally across columns.

I've attached an example of the data I'm working with. The green cells are for the criteria selections and the red for the formula.
