Looking up a Value Based on Two Criteria

Hi All,

I'm struggling to find the right formula to build my report. The report is intended to populate financial information based on a selected site. I have a lookup table that lists financials either actual (prior to current month) or forecast (>=current month). The trouble I'm running into is that the lookup table has the dates as column headers, while the report has the dates in a row format. I've attached a couple of screen shots below showing both the lookup table and the report.
