How to display particular digits from a text value, with one formula?

I have some numbers formatted as text to have a leading zero for particular values. I need to display particular digits as follows in the attached workbook. In the workbook, I use a mix of LEFT and MID functions, though I was wondering if I could have one formula being I work with different values?
