Formula To Index All Unique Values Found Based on Criteria

In sheet two of the attached, I'm looking for a formula to return all the unique item numbers from column A in the data tab where 1) the item in A2 has a qty >0 in column G, 2) column H in the data tab has a value greater than 0 and 3) where the both items were on the same PO from column C in the data tab. I've been trying to think through the best was to find a unique identifier to sumif off of so that is what is in I through K of the data tab. Thanks in advance. Think I've fried my brain on this one and missing something probably simple.
