Formula to Show the Reporting Month Instead of the Actual Month Based on Week Number

I am not sure if this is possible, but in the sample file with sample dates of November and December, I need a formula in the Reporting Month column to show that if the week number equals to ?5?, the reporting month will show as ?November? and if not, it will show as ?December?.

I want the formula to be applicable in all months so technically, week #5 belongs to the previous month even if the actual month is already the current month.

I hope I make sense. Thank you in advance!

Maybe include a sample with more examples and put in your EXPECTED results. Does the "created by" column come into the logic?

I revised the attached file. The date in the Created By column is manually entered. I said in my previous message that week#5 belongs to the previous month even if the actual month is already the current month, but I realized that there are months that have five weeks. The week number resets to 1 every time the first Monday of the next month is reached. Hope this helps!

I'm sorry. I didn't read your question properly.
This question is different from your previous question.

The formula for the reporting month is (custom format cells as mmmm):
    Formula: copy to clipboard

or as text (no format needed):
    Formula: copy to clipboard

Thank you so much for this! My only concern is for the week of November 7, 2022, it shows as week number 2 instead of week 1.
