I want return value in coloum time based on data in left table like i did in table 1, but i cant use same formula for table 2,

Try this:
=INDEX(E:E,MATCH(1,(H9=LEFT(B:B,LEN(H9)))*(H10=LEFT(C:C,LEN(H10)))*(H11=D:D),0))
You'd need to remove the asterisks:
