Data Validation Lists and dependant formulas HELP!

Im having a terrible of a time building a form using variables that create dependants based on data validation lists. Anyone know a formula to match criteria based on selections from data validation tables?

Essentially, if select a name in the validation drop down for an employee, I am trying to get the employee number to pull up based on the employee name or vice versa. This will be the staple for pulling the remaining data and seems to be a all day roadblock for me. I have tried to use both the match and the oset function using named ranges and I keep getting an error and cannot trace the actual error. Even when using the list that the validation table is based on, Im still having a itch of a problem. Does anyone have ANY help?
