Index Match Multiple Colums Headers

Hi
Having a problem with the attached file, need some assistance please.

On the data report I need to match two headings and then the row to pull the relevant data onto the summary sheet, currently it is pulling incorrect data and I also have an n/a.

This forum is fantastic, I have learnt loads over the last couple of months as I had very little experience with excel, thank you to all the posts and giving me great answers to all my queries. This is the first time I have been unable to find a solution to my problem and hoping someone can help me soon.
