Consolidate Index/Match formula into variable range

I've been hacking away at this formula for awhile, but outside of what I have now, I can't seem to work out how to consolidate the Index portion.

I tried to use the same Choose(Match configuration in the Index portion, but I kept getting the error that there was a errror within the structure.

    Any thoughts?

    The workbook might help.

Maybe I'm not fully awake yet, but the final part ,0),"") doesn't seem right. The zero appears to be an orphan in that it doesn't belong to anything.
I suspect you intended it to go with the initial MATCH but it is outside the brackets for that MATCH.

    Thanks Bob.

I am using 2007 even though the extension shows .xls.

The referred to formula is in C4 (PPT Tab) with data validation in A1 & E1 still on the PPT tab.

You must be more awake than I, that worked. I ended up with this and it works for my needs.
