How to get date needed using index and match or any formula?

How to get date needed using index and match or any formula?
Any help? From everybody.
I prefer to use CONCATENATE myself. In a new column use CONCATENATE to combine multiple columns which will create a unique conbination for each and then you can do a simple vlookup. Index Match probably works but quite frankly is a huge pain.
Could you make formula using concatenate? I didnt familar to this. Thanks
