    Index and match with multiple criteria

    I need your help for the attached table with question.

    Date needed-----category-----factory-----code-----date cast

    Jan 1-------------column-------A1---------CC 001---feb 1
    Jan 2-------------column-------A2---------CC 001---feb2

    Question:
    How to get date needed using index and match or any formula?

    I prefer to use CONCATENATE myself. In a new column use CONCATENATE to combine multiple columns which will create a unique conbination for each and then you can do a simple vlookup. Index Match probably works but quite frankly is a huge pain.
