Multiple IF Functions or VLOOKUP functions

I have a data entry page for all the equipment in our building, problems with the equipment and the date. I am then trying to make a sheet that brings all of it together. It would display the date, then the equipment that is getting fixed and the issue it is having. I tried using IF statements and VLOOKUP but could only get 1st column data. Also I tried putting the date on top rather than on the far left column and not sure which is better. I uploaded the sheet for any help anyone could give. Thanks!

Thank you so much! Do you think there is anyway that when I enter something an issue with a forklift it can automatically put the forklift number and date as well in the outstanding repair page?

the cross-tab format that you are using to capture data is going to put pressure on calculation / performance. see the attached method, which uses a helper column:
