    Sickness Absence Monitoring Form

    Hi

    Im trying to create a form that populates a spreadsheet so that I can monitor absence levels effectively. This form will be completed once I have received an absence form from the employee, and hopefully I will be able to go back into the record and update this when they return to work.Absence Form Data.xls

    I have attached the spreadsheet showing the fields I want on the form, and the drop down menus that are needed for each field

    Any help will be really appreciated

    Sorry--what's your question? Your spreadsheet has several columns of data. You said you are trying to create a form but there is no evidence of a form in this file. Are you saying that you want someone to create a form that has all these fields to fill out for one particular absence, and then the data on that form is added to the last row of the spreadsheet? Not particularly difficult but will require macros.

    Your dropdown fields refer to data that is in a different file, so we can't see the values that you want to allow, and these names can't be used in a combobox to populate it.

    Hopefully the attached will get you started. You will have to make modifications to get the comboboxes to work the way you want.

    Can Working Days and Hours Lost be calculated from the dates, or do those need to be entered by hand? Here they are calculated based on elapsed working days. You will have to make modifications to take into account holidays, or work schedules that are other than M-F.

    Which fields are required and which ones are optional?

    You should review the code and make modifications based on your answers to the above questions.
