    Summary Worksheet that pulls data based on a date match

    I am trying to create a summary page to use with a worksheet of data. I would like to be able to enter a date on the summary page and have it automatically draw name, email, last 4, and provider name data and populate on the summary page if the date entered matches any of the dates in the RSR/TR columns. The file is attached and is much more clear than my typing is.

    Any help would be greatly appreciated!!

    By adding a couple of helper columns, the attached should work.

    Try this Array Formula (Cntrl+Shift+Enter,not just Enter) In A7:

    You could run into issues with the solution provided if you have the same date listed twice in one row.
    To avoid this, replace the formula in the "Data" sheet, in cell A2 with the following (and drag down):

    Melvinrobb - this is awesome and exactly what I need. I won't ever have the same date in one row...this might be a lame question, but how do I extend the formula down to add more lines of data on the data worksheet...I added new rows and it shows the count if the dates match but it does not carry forward to the summary...I am sure I left out something simple. Thanks!

    In the formula, you need to change the range it looks to encompass the entire area. Right now it is referencing rows 2:4 in column A. I would suggest either changing the range to a range larger than you think you're data will go (ex. $A$@:$A$1000), or simply insert above the final row when you need to add new rows, as your formula will change to $A$2:$A$5 if you insert a row above row 4, $A$2:$A$6 if you insert two rows, etc.
