Index / match 2 criteria row and column containing date - please help!!

Hi everyone, first time post here but been a member for a while. Usually managed to sort my queries by reading old posts but this one has got me.

I have a set of raw data on one sheet that has an item name down the first column. On the top row I have a set of dates separated with a number of spaces.

In between these two axes I have a number of data and I am trying to isolate using the Index/Match function in another sheet.

For example in the below pic, the cell I put formula into on the next sheet I would want to match up the name and then the date and where this point meets, retrieve data in red circle.

Please take a look at the attached spreadsheet to see what I am doing wrong as it just WONT work for me.

The formula I am trying to use is this;
=INDEX('CSV data'!$B$3:$ZZ$1000,MATCH($A6,'CSV data'!$A$3:$A$1000,0),MATCH($C$3,'CSV data'!$A$1:$ZZ$1,0))
