Need help pulling duplicate data to one worksheet

I'm hoping there is an easy solution to this and I am just over thinking it all! I've attached a sample document of what I am looking at.

What I am trying to do is enter data into the 'Hospital' sheet and have any duplicates also show up on the 'Readmits' sheet. Ideally, I would like the entire row to pull to the 'readmit' sheet but truly the most information I need pulled are the Name, Admit Date, Actual DC Date, and Month columns.

For example, Alice Smith would pull to 'Readmits' three times and Joe Smith would pull two times.

Hi,

Do you need them in any specific altered order or just as they are?

If the latter then you could simply click in a cell on the readmits sheet, type = then click on the relevant cell on the Hospital sheet.
Then drag copy that formula as needed.

Perhaps that's too simple a solution and I'm overlooking something fundamental in your post...
