Formula to get cell value under 3 diffrent conditionds

Hi All,

I need one forlula which gived the value of the cell depending in 3 diffrent conditions.

There are 3 colomn 1st is Name 2nd is Date 3rd is login date.

so in other sheet i need date as header and login time as per the name.
Name 1/3/2013
