Return Headers Based On Two Values

I am working on Vendor Rebate Schedules and I need to somehow translate vendor's schedules into our own upload-able schedules for purchase systems. I need to return Months based on two values ProductCode and Percent available for those months. How is it possible using Index and Match fucntion?Attached file has two tabs VendorSchedule and InternalSchedule and I need to return Months on the InternalSchedule.
