Matching multiple values in different rows and columns

Hi,

I'm unsuccessfully trying to make up a formula that matches several values, which are not in the same row/column.
What I'm trying to accomplish, example from attached file: in Sheet2, K9 cell value should be taken from Sheet1 cell C4. So it should match "Name1", date: "2011-11" and activity "1.1.1.", but I'm lacking excel skills to make up such a formula.

Any help would be appreciated.

Simply used index with two match functions inside and added +1 to retured value from match formula.
