Column number

Hello:

Please refer to attached sheet.

I have date in column A and i need a put column reference is column B as shown.

For 1st Date : Column E
For 2nd Date : Column I
For 3rd Date : Column M
Basically add 4 column for every date.

Please help to fill up column B as explained.
