Return values from multiple rows (with unique identifier) into single row - Arrays/Index?

Hi - would appreciate help with this problem - refer to the attached spreadsheet.

I have an output from a database where each person has a unique identifier but their related data e.g. name, address, description are spread across multiple rows, with some spacer rows that contain no data.

I would like to sort into 1 record per line as per the attached highlighted in yellow.

    Thanks very much!
