How to automatically add a date to a dynamic list of dates from another cell in the spreed

I am trying to create a list of all treatments received by my patients. On my master Overview page, I input the new treatment date, it is then automatically forwarded (pulled actually) using ="Patient 1"!M4 on the individual patient sheets into a cell (D4). Column A is a dynamically expanding list down the column that captures these treatments, but i cannot figure out how to connect D4 to the empty row below the list in Column A as it grows (actually causing it to grow). Any help would be grateful. I am not a computer wizard by any means.

A sample workbook would be more helpful to see the arrangement of the sheets and data.
Meanwhile you can try using the index and counta to get the reference of the last value entered in the date column. Like...

Assuming column A is one which you are entering new treatment dates.

    Hope that helps.

Where do I put the formula? in d4 I already have one to pull the information from the overview sheet to the individual sheet in d4 (='overview'!m4). I want that value that is pulled into d4 to also be added to the end of the list in column a as a new treatment. Column a is already set up to expand dynamically.

Re: How to automatically add a date to a dynamic list of dates from another cell in the sp

I am a bit confused... I am not trying to get the reference of the last date, I am trying to automatically add the date to the dynamic column from an automatically filled cell. Like this:
    A B C D E F G H
row 2 Treatment Date # pellets 1mth Lab 3mth lab Last Tx Nxt Tx Last PSA & T Draw
    row 3 4/11/2013 15 2/13/2014 4/14/2014 1/14/2014 6/13/2014 11/11/2014
    row 4 7/23/2013
    row 5 10/24/2013
The date in F3 is filled with ='overview'!m4 automatically when I fill it in on the overview. Column A is a dynamic list already, I can manually put new dates at the bottom and the list expands. What I would like is that F3 both pulls the date from the overview sheet and posts this new treatment into the dynamic treatment column A, without overwriting prior entries, but instead creates a treatment date history. If I cannot do it with F3 I am very open to ideas.
