Attached is a screenshot of what I need help with. Basically I need to know that if PersonA has say 13.74 CPH and $776 DPH - what the incentive would be ($575)...

A picture of a spreadsheet isn't very useful. Suggest you post a sample workbook.

Uploading a sample workbook, rather than an image, is a better idea.
Then it is possible to construct a formula in the sample file, and test it.

Probably
=INDEX(interior of table,MATCH(in rows headers),MATCH(in columns headers))
would do the work, but retyping your table to find exact formula would be silly work.
