Looking up multiple criteria and returning a value

Would love some help in trying to find a match when entering some data. Under the 'match simulation' tab, I am hoping to look up the value located in A1 in the 'data summary' tab and return the matched value. For example, I am hoping that in C3 of the 'Match Simulation' tab, the value of 96.5 (from 'data summary' X4) will be returned through locating, 'Adelaide' (A1), 'AAMI' (B3) and 'For' (C3) from the 'data summary tab'. Hope this makes sense...any help would be greatly appreciated!
