checking boxes....

Hey everyone,

I'm creating this template for work. In a box I was wondering if I had it where they marked "yes" I could somehow make another box pop up and explain why they marked "yes". If someone could point me in the right direction that would be amazing. Thanks in advance I appreciate it.

This can be done using an IF statement. Please upload a sample template with explanation showing what needs to reflect when a cell is marked "Yes".
