Avery Label RTF to CSV in Columns

Avery Label RTF to CSV in Columns:

I have a RTF file that has 34 pages of:
    Line One: Name
    Line Two: Street Address
    Line Three: Zip Code
    Line FOur: State
    Line Five: City

How do I import or sort the 5 lines that are repeated for 675 addresses in to a CSV or excel file with columns that the printer can use for data merge when printing newsletters???

    Please help!

    many thanks
