INDEX MATCH across multiple rows and columns

Hi all,

I'm sure a problem similar to this has been posted elsewhere but I can't find it. So I hope someone can help me.

I'm trying to determine a formula that will output the following information (B14) based on the inputs (B9 to B12). I think it requires an index match but I'm not sure how. Also, please note that I want to be able to enter the inputs separately as follows:

To attach a Workbook
(please do not post pictures or links to worksheets)

• Click Advanced (next to quick post),
• Scroll down until you see "Manage Attachments",
• Click that then select "add files" (top right corner).
• Click "Select Files" find your file, click "open" click "upload"
• Once the upload is completed the file name will appear below the input boxes in this window.
• Click "Done" at bottom right to close the Attachment Manager.
• Click "Submit Reply"
