How do index match exact corresponding to the range against of multiple reference.

i have changed the data in single sheet that is two contain of Name Table 1 and Table 2.

what i want i have mentioned in yellow color.also duplication are indicated in red color.

Table 2:item No-17231 that contain are three time repeat of same range.

That is.

But my Query is:

whatever i have mentioned the value corresponding to the range in table 1,the same was return the value against corresponding to the r

ange and reference.
