VLookup to return all results from 3 columns in another sheet

Please see the attached sheet.

I'm tyring to use vlookup in "City Driver & Contacts" sheet to compile the results from 3 columns in the "Move Polling 11-2015" sheet.

In "City Driver & Contacts" sheet cells K4-K6 I'm wanting to get a yes, no, or maybe from the match in Column "D", the info is in ""Move Polling 11-2015" sheet cells C50:E52.

Is this possible? I have tried different combinations of vlookup, match, index.

Any help is greatly appreciated.

Did you attach the correct workbook? I'm not seeing the sheets you describe.
