Monthly PTO Accrual Recon

Hello,

I am in need of a spreadsheet that I can calculate the dollar amount that needs to be accrued for PTO balances. I need a chart that will have the name, dept, salary amount/or hourly rate, accrual rate, PTO balance, used, and the remaining balance, with the dollar amount. Does this make sense? Please help. This would be used for month end recons.

Is there something that shows this amount in dollars? So it takes the balance and times the pay rate?
