Linking Forms to Auto Populate Excel

Hello, everyone! I'm pretty new to excel and am not sure if what I am wanting is even possible so any guidance is greatly appreciated!

Is there any way to take a form (probably created in Word) and link it with an Excel Workbook so that the information is automatically populated?
