I'm guessing you need something like this:
Formula:
=INDEX(Sheet1!$D$2:$D$5,MATCH("9865"&"Old-Link",Sheet1!$A$2:$A$5&Sheet1!$C$2:$C$5,0))
committed with Ctrl-Shift-Enter rather than just Enter as this is an Array Formula.
If the data you need to search for is in cells, say, A1 and B1, then the formula looks like this:
Formula:
=INDEX(Sheet1!$D$2:$D$5,MATCH(A1&B1,Sheet1!$A$2:$A$5&Sheet1!$C$2:$C$5,0))
again, committed with Ctrl-Shift-Enter rather than just Enter as this is an Array Formula.
An Array Formula is confirmed by pressing CTRL+SHIFT+ENTER to activate the array, not just ENTER. You will know the array is active when you see curly braces { } appear around your formula. If you do not CTRL+SHIFT+ENTER you will get an error or a clearly incorrect answer.
