Userform send text box to active cell

Dear Sir / Madam,
i am attaching a example XL sheet with a userform and i need help populating the ACTIVE CELL with data from the Userform, however the data should be concatenated with the following information:
NJ:N=40,V6=30,V9=20

Secondly i had researched and inserted macros to launch this userform when Ctrl+H was pressed. the macro worked but when i restarted the XL sheet it stopped working or it was sending me to another XL sheet. any help on this macro will be appreciated.

many thans fo ryour help, it works like a charm. 1 small question:
can you please amend the code so that i can see the existing values in the cell in the active box.
the NJ values should show in NJ boxes. and UV values in the UV boxes.
and if i change the values in the box they should then change the active Cell. the purpose is to show any existing values so the user doesnt overwrite them by mistake and payes attention to them
