PDF/PRINTABLE Document Prep template

Okay so here is what I am attempting to do:

-I have a PDF file (fill-able)
-Need to insert this PDF into an Excel Sheet, so that when the "Input" Sheet is filled in (separate sheet) , it automatically populates this PDF and can be printed and or saved as a PDF.


So,

Excel File with 2 sheets.
Sheet1 is for inputting data (EG: Name- / Date- / Etc.)
Sheet2 will be a PDF document, which will draw its fill-able data fields from Sheet1

Thank you. I appreciate you taking the time to help/respond. My only follow-up questions to this are; regarding the re-creation of the PDF - Do you know offhand what the format/ settings are to be set up, manually or not that will "pre-format" or what have you so that it is recreated at the exact same size and view when printed or saved? I know to set up page breaks and whatnot, but I have tried a few times to do this and it ended up printing/ saving as a PDF out of format or mid-sized. Just wondering if there is a shorter process rather than having to play around with it many times? Either way, I appreciate your help! Thank you very much!
