Cannot convert VLOOKUP equation into a range equation

I cannot seem to convert the table equation to a simple equation for a range.

A colleague of mine created a lookup equation which retrieves information from an index range on a separate sheet which seems simple but I cannot seem to get it to work. What am I doing wrong. I have removed duplicates in the look up column.
I am trying to standardize the name original name of post secondary institutions into a consistent format as many have spelling mistakes or written differently i.e. University of Waterloo and Waterloo University.

Your help would be most appreciated.

    Re: VLOOKUP problem

It looks like maybe the data on the Masterprobl sheet was in an Excel Table, but now it is not. I put it back into a table and it worked.

Re: Cannot convert VLOOKUP equation into a range equation

Thank you. Your solution worked as it was clearer in a table format. I reformatted the whole range and it seems to work on the attached file. I will need to see if it works on the full spreadsheet. Many thansk
