Partial match then return value from a different cell

Hello, I'm sure this is really easy, but I'm not having any luck with figuring out the correct formula to get the desired results.

I'm trying to compare two lists in separate sheets. If there is a partial match in 'Main' Column A (from 'Database' column D), then return a value in a different column on the main sheet. I've tried using VLOOKUP and INDEX, but can't figure out the right way in order to get it to work.

I've attached a very simple sample file. The first 2 worksheets (Main & Database) are samples of what I currently have. The third worksheet (Desired Result) is how I would like the first worksheet to look once I get the correct formula in column C/D/E on the Main spreadsheet.

I changed column D on the Database sheet to use a formula:

The I used the following formula in C2 on the Main sheet:

Copied the formula across and down. Is that what you're after?

Just curious, if I want to add the title and acronym to the main spreadsheet what do I need to change in the VLOOKUP formula?

In which case, change the formula in C2 to this:

Then copy across as needed and put the column names in row 1. See attached.
