Extract information based on multiple criteria and also date range

Hi,

I am trying to extract information on sheet to another sheet based on multiple criteria (More Than 3) and also data should be pulled between two dates.

i felt this should be use full but getting "#NA".

In DataBase sheet, try to mark #order numberin column A like this:
In A3:

then drag down
Log sheet-1, A5 drag down to creat #order number list:

In B4:

Drag down and accross
