Possible to classify rows of data by ID into 3 Groups based on column data input.

Wondering if it is possible to classify my rows of data in my table into three categories based on data recorded in two columns, after doing some research it looks like I may have to do countif and if function.

The spreadsheet attached shows the format of the data currently and how I want the second format to look like.

Here is how I would like the data to be classified based on what been input in the c Appointment and N Appointment columns.

For a particular ID if, the c appointment and the N appointment both have marked as 1 then I want C then N.

If N appointment is only marked in the column then n only.

If possible if the N appointment is marked as 1 on a earlier date and then c appointment is marked on a later date then, n then c this could be based on the date column so - I understand this one would be difficult...
