Importing text mcq questions to excel table

I hope this message finds you all in good health and high spirits. I?m writing today to humbly seek your expertise on a topic that has left me somewhat perplexed.

I need to import questions from a text files and the format of the questions is as follow

I've attached the excel file that I need the questions to be imported into. and added a question manually in the first row.
Hope you can take a look and guide me how to fix this. (I have many text files with100s of questions)

Thanks for your msg. and sorry for not making things clear for the second time.

The questions was exported from our college old database. I think it was MySQL tables
and when exported it came this way. just 6 lines
1st line is the question
2 to 5 are the choices
6th line is the correct answer

the excel file is to import the questions to another application.
and this format was provided by the software.

I don't need the category or explanation columns, however i can't delete it from the file. so i'm leaving it as uncategorised

as mentioned thr excel file was supplied by the new application as a guide how the questions should be construed.
they had 3 questions as guidance for us to fill it.
I changed the first one as you can see in the attached file. but forgot to remove the 2nd and 3rd.

TLDR.. its just example to fill in the file.
