Excel Grouping

I am attempting to group a bunch of information on a sheet and not all plus signs are available after creating my groups. For instance, I'm trying to create 3 groups in one section, but only 2 plus signs generate. What am I doing wrong?

Try inserting an empty column (or row) in between.
