Three-way lookup with Merged Cell reference

In the attachment, I have a result sheet and a source sheet. I would like to create a lookup formula based on the source sheet to match Employee Name, Type of Forecast, and Work Order. For instance, for Tya Marie, Original Forecast, and Work Order 60051669-BIM, the lookup should be performed on the source sheet.

    Can anyone help me please

If your result sheet is just a mirror of the source the formula bellow will do the job.

    In G3
    Formula: copy to clipboard
