Timeline/Schedule help

I'm trying to create a timeline for scheduling purposes. I want it to look like the image file I have.

And I have created one similar to it in the excel file I've attached.


My question is: How do I have the diamonds correspond to the start and finish dates for each line, and how do I add the line that attaches in between the two diamonds?


I tried creating a chart, but it's not coming out like how I want it.
