Pivot table field name not valid

I need some assistance, when I am trying to create a pivot table it pops up a message stating the field name is not valid. I have done everything I was instructed to do in order to capture all the information needed for the pivot table.

I started with cell A1 and selected the range needed. I am trying to teach myself how to do pivot tables and I am not very successful at it.
