Create new rows based on multiple cells on single row

Hmmm... that probably isn't the catchiest thread title in the world but hopefully it sums up what I'm trying to achieve.

Basically I have created a Quality Control workbook that logs errors in lots of different categories for items a user enters into a system. We currently have two teams performing this Quality Control check and therefore I have to reconcile both sets of results to see where they match and more importantly where they don't. From this sheet I am trying to generate an action list for another user of each of the mismatches they need to investigate.

However this is where I run into difficulty. I can't find a way to have multiple new lines generated on a new sheet if a certain error code (anything greater than 2) is returned in cells on the original sheet.

In the attached file you will see that there are 4 errors on the first line item. I would like this to generate for lines on the "Action List" sheet and for columns I and J on the "Action List" sheet to reference which field was in error. (e.g. Type B - Q2).

Any ideas on how I could tackle this?
