You have some sort of BEFORE/AFTER sample sheets we can see? Click GO ADVANCED and use the paperclip icon to post up your sample workbook.

Which version of Excel are you using? It would help if this was entered in your User profile

Excel 2007
1) I made your lists on the hidden sheet dynamic so if you add/subtract from them, the drop down selection cells will still show the updated options.
2) I put the drop downs into standard in-cell drops for simplicity.
3) I assumed the sheet to be created is based on the code in E8.
4) I attached the following macro to your button:
