=IF(AND with VLOOKUP with multiple criterias

I have a work sheet that I have figured most of the formula on, but now excel says I have listed too many arguements. So I had to stop the formula. It is not completed. I am trying to figure out how to get the value in N2 from the rates that I have listed on my rates sheet through VLOOKUP. I have half the formula listed in cell N2. But it will not let me go any further. What I need it to do is return the rate listed on the rates page if J2, L2 & M2 are listed. I have gotten 1/2 of the formula to work but it will not allow me to list for Employee class level C or D, the medical plan or election to get the totaled amount. I was able to get the class level A & B to work, just not C & D

It seems to be working for me? Maybe I am not understanding what you are trying to do.
