Best way to filter and retrieve data from spreadsheet

The various locations within my agency submits safety reports to me via a Google form, which puts the data into a Google spreadsheet. The form can be used for multiple report types (Fire Drills, Earthquake Drills, etc.) and the data from each submission populates a unique row within the spreadsheet and is time and date stamped. On a monthly and/or quarterly basis, depending on the type of report, I need to identify which locations submitted their various reports for a given time period. I need only to know if the report was submitted.

For instance, if I want to see if our South location submitted a Fire Drill report in the month of June, I would need to go to the spreadsheet and first narrow my search to June submissions, then narrow my search to only reports submitted by the South location, then determine whether any of the reports South submitted are Fire Drill reports. For this particular task, I need a formula that will simply return a yes or no answer.

How would this best be accomplished?
