    Index Formula: 2 Criteria Help

Greetings everyone this is probably just a simple Index / Match formula, but I have been trying to apply the examples of others to my problem and I'm not having any luck.
I have attached an example of what I need.

I need the formula to behave like a V Lookup with 2 criteria.

My data is organized in rows.

The first criteria is a column with months of the year. The second criteria is a column with part numbers. When both of these criteria are requested I want the formula to return the corresponding price from the 3rd column.

I also have a description in my excel sheet to illustrate what I am looking for.

    Thanks for your help!
    Chris

    Hello,

    in Q11

    =INDEX($E$5:$E$17,MATCH(O11&P11,INDEX($C$5:$C$17&$D$5:$D$17,0),0))

    cheers,
